Good Faith Estimate Notice (No Surprises Act)

Under the No Surprises Act, you have the right to receive a “Good Faith Estimate” explaining how much your medical or mental health care will cost.

If you are uninsured or not using insurance, you have the right to receive a written estimate of the expected cost of services before receiving care.

You can ask your healthcare provider for a Good Faith Estimate before scheduling a service or at any time during treatment.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill.

Notice of Privacy Practices

Your privacy is extremely important. This practice is committed to protecting the confidentiality of your protected health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA).

A full Notice of Privacy Practices outlining how your health information may be used and disclosed, as well as your rights regarding that information, is provided to all clients as part of the intake process.

Clients may request a copy of the Notice of Privacy Practices at any time.
